Celebrity Golf Tournament

Please register for the MULBA Mid-Atlantic Beverage, Food & Lodging “Stroking For Scholarships” Celebrity Golf Tournament, Monday, September 19, 2005 by registering and paying here or printing and returning the completed entry form along with a check or money order by August 15, 2005. The cost is $150 per person for golf cart, greens fee, range balls, lunch, goodie bags, prizes, non-alcoholic beverages. Solo golfers are welcome as well as two and foursomes.

Want to golf with a Celebrity? By donating an additional $100 (per person), you can play with the celebrity golfer of your choice.

Of course, non-golfers and golfers' significant others are welcome to join us as spectators. You are welcome to join us for our lunch and awards program. Lunch may be purchased for $20.00.

	If you are registering as an individual please only fill out the first golfer area of the form.
	If you are registering as a group please fill out the team name and all golfers in your group.
	If you want to play with a celebrity please indicate your first and second choice for the celebrity of choice.
	Foursome: $600
	Single Player: $150
	Celebrity Golfer - $100 + Single Player Fee per player (1st come, 1st serve, list subject to change at tournament discretion.)
